𝗘𝗱𝘂𝗰𝗮𝘁𝗶𝗼𝗻𝗮𝗹 𝗔𝘁𝘁𝗮𝗶𝗻𝗺𝗲𝗻𝘁 𝗶𝘀 𝗻𝗼𝘁 𝗿𝗲𝗾𝘂𝗶𝗿𝗲𝗱. 𝗘𝘃𝗲𝗻 𝗛𝗶𝗴𝗵 𝗦𝗰𝗵𝗼𝗼𝗹 𝗚𝗿𝗮𝗱𝘂𝗮𝘁𝗲𝘀 𝗰𝗮𝗻 𝗮𝗽𝗽𝗹𝘆.

1. Fully-accomplished Application Form

2. Passport-Size ID (4 copies)

3. Valid ID (Original and Photocopy)

4. Examination Fee - 500 pesos.
